| Abstract 
              -- The primary aim of this project is to establish 
              a medical system model that will maintain community residents’ health 
              data during ordinary times, strengthen horizontal and vertical integration 
              within the medical system, realize a diagnostic consulting system, 
              and employ team cooperation to provide the most effective medical 
              care to the public. When a disaster occurs, information in this 
              system can be used at the earliest possible moment to provide residents 
              of the affected area health materials and medical resources. It 
              will enable the emergency response center in the disaster zone to 
              fully understand the situation on the ground, so that it can supervise 
              rescue and medical work and swiftly achieve comprehensive emergency 
              relief results. This project, which takes the residents of the Chushan community 
              in Luku as its subject, will be developed jointly by National Taiwan 
              University, Chengching Hospital in Taichung, Tzushan Hospital, Luku 
              Healthy Center, and the civil authorities of the Luku area (Chen 
              Shou-chih's Clinic, Wang Hung-chou's Clinic, and Luku Farmers Clinic). 
              Specific tasks include the establishment of a community-centered 
              health information system, the realization of two-way patient-referral 
              service procedures, the provision of channels for continued education 
              for grassroots-level physicians, the establishment of horizontal 
              and vertical cooperative models for medical organizations at all 
              levels within the system, and the control of manpower and material 
              resources within the system that can be mobilized in the event of 
              an emergency.
